In addition, the pages and the brochure My health and rights as a teenager, what I need to know in French and Hausa were installed on the tablets. After two days of training, provided by the CulturePlus association, the use of the material was well mastered. This new tool is proving to be valuable during the facilitators' visits to the villages.

Now the screenings can take place during the events for which the four facilitators travel by motorbike, in the villages of the four rural communes of Dankassari, Dogondoutchi, Matankari and Dogonkiria, with a total population of around 300,000 inhabitants. Previously, the screenings that were organized required a vehicle, a projector and a small generator. The cost of these screenings made it impossible to do them very regularly.

A supervision mission to see how the events are going was organized. Here is the report we received : "On November 25, 2024, we went to the commune of Dankassari, in Goubey, where the women were grouped together at the CSI with the participation of the staff and trainees from the national health school who came for their annual internship. The meeting was very rich in interventions where the trainees had the opportunity to participate. We saw an audience of women and men and even children, very interested in the videos or comments and other interventions were rich in lessons. In conclusion, the tablet operation is a total success, because it was very well received by the communities.

Following their 3-day training session on family planning, male and female monitors unanimously wished to benefit from more training sessions, so as to review former knowledge and if necessary learn new elements. They also wished to have access to tools and necessary material to illustrate their message.

Consequently, AECIN and AESCD suggested illustrated pagivoltes, which were ratified by RAEDD, created and delivered to communitary relays for their training.

The aim was to translate the 5 main messages stated by Tarbiyyya Tatali concerning the importance of mastering fecundity (not only spacing out births but also mastering the size of the family), which had to be conveyed to villagers by appointed relays.

The importance of safe deliveries: modern birth-control devices and spacing out births improve mother and child's health as well as the family's standard of living.

Refusing early marriages before 18 prevent under-age mothers' deliveries which jeopardize their health and their children's health; it improves girls' school attendance.

The importance of affective and nutritional caring devoted to each child: better to have fewer and to attend to them well.

Children's future is better insured if the family isn't too large, the ideal family is that where parents can care for eqch child concerning their health, their education and their nutrition.

Women's self-sufficiency is improved thanks to jobs generating an income if she doesn't have too many children.

AECIN and AESCD have looked for the most suitable pictures, particularly through the numerous documents indexed or published by Tarbiyya Tatali: ‘Social and Personal Education booklet', ‘My Health and Social Education' booklet, ANBEF document, CEDEF, and a new drawing showing a town council including women and men.

Overall, 27 illustrated charts (pagivoltes) are used to illustrate the main topics dealt with at the training sessions, with legends translated in Haoussa.
They have already been tested during supervision fieldwork missions in Bawada-Dagi (Dankassari), Batanbari (Doutchi area); Montere and Mailo, (in presence of Aziz, a photographer who could take photos about the use of the new leaflets by relay women and Maimouna Kadi, the counselor).

They have since been given to all the monitors (women and men) trained in both Dogondoutchi and Dankassari as part of the family planning project.

In 2024, the four family planning facilitators as well as two managers of RAEDD and RAIL were equipped with digital tablets containing the pagivoltes.

A 60-page booklet written by RAEDD with financial support from Ville de Rennes and AECIN.

Contents:

From childhood to adulthood, what is puberty? Boys' puberty, girls' puberty.

How to remain healthy: What is health? Bodily hygiene. Hygiene in dwellings. Food hygiene.

Social relationship. What is love?

Transmitting life: What is having a baby? The stages of pregnancy. Birth.

What is Family Planning? Unwanted pregnancies.

Children's rights. Under age marriages.

The booklet has been carefully prepared. The male and female authors and the illustrator, all Nigerien, were thoroughly involved, so that the booklet is adapted to the local situation and clearly aims at preventing early marriages, it insists on family planning (in accordance with the Nigerien Government's recommendation).

A training session on the booklet involved 21 participants (11 men and 10 women), teachers in Matankari and Dankassari secondary schools, as well as family planning social workers who visited the villages and communes of Dankassari and Dogondoutchi in December 2017. The training session was held by a woman and a man. The participants insisted on following up and extending the project, particularly by publishing versions in local languages (haoussa locally). Male and female teachers and family planning social workers welcomed the contents of the booklet. It is considered useful for both training and teaching. It also raised discussions; namely as concerns the necessity for teachers to be blameless in their attitude to girl students.

A second training of 24 people on how to use the booklet took plane in june 2018, for all the secondary school professors in Biology from Dankassar and Dogondoutchi, twenty two men and two women.

A training session on how to use the booklet had been planned late in 2020, it was postponed by the Nigerien Government as all meetings were prohibited owing to COVID. It eventually took place from March 12th March 14th 2021. The coaches were Geneviève Courjan and Mahamadou Roro, from Niamey, both members of the booklet's editorship. It had been planned to train 15 women and 15 men, eventually 14 women and 16 men were trained. 600 more copies were printed and handed out to the participants.

The booklet is shown in the film 'A girl trio in Dankassari', it aims at explaining what an early marriage is,

The massive population growth, higher than economic growth, is prejudicial to the Nigerien population's well-being.

Family planning in Niger

Modern contraception is still little used in Niger, particularly in rural areas.
The Public Health Ministry has adopted a long-term plan (2012-2020) to contribute to curbing population growth through family planning. The goal is to move from 11% to 50% prevalence of modern contraception through three strategic lines reinforcing family planning:

increasing supply in health centers

increasing demand through education and awareness

promoting a suitable environment.

Its implementation requires the mobilization of significant resources. Family planning awareness is part of a broader project on maternal and child health.
This project is co-financed by the Britanny Region, CD 35, and the city of Cesson-Sévigné. SDE35 supports the electrification of health centers, while AELB and CEBR fund the sanitation component with the construction of latrines in integrated care centers and health centers.

Despite the coup d'état on July 26, 2023, and the suspension of all aid from the French State, local Breton authorities have chosen to support all ongoing actions for the 2023-2024 period.

The counsellors of Tarbiyya Tatali

A counselor was engaged in November 2015, out of AESCD's funding. She is in charge of increasing demand through education and awareness. On the other hand, health centers are in charge of answering demand.

She first got in touch with the ‘Savings For Change' groups groups which are still very active in Dankassari despite the Stromme Foundation's decision to stop financial aid.

She visits all the villages in the Dankassari rural community for awareness reunions. She aims to identify and train correspondents in the villages.

The first goal is to combat early marriages (before 18 years) and first pregnancies occurring before 19. The second goal is to improve women's and children's well-being by spacing out births and using modern contraceptive methods. The third goal is to contribute to an evolution of mentalities, particularly among men. The fourth goal concerns population awareness, establishing the link between economic and human development in Niger, and mastering demography.

A second counselor was recruited in 2018 for the villages of Dogondoutchi, with financial outgoings being shared by AECIN and the Orsay Dogondoutchi Exchanges Association with a similar mission

Training, pagivolts, and video sketches

Trainings

Training of relay people is organized for the women and men identified by the two counselors. Initially lasting three days and devoted to modern methods of contraception, they were extended to four days to also address the issue of girls' schooling. A three-day training reinforcement is also organized, to exchange feedback on their practice in the villages and to check that the previous knowledge has been assimilated.

In Dankassari, the training sessions were financed by the AESCD within the framework of the program "Progressing towards Sustainable Development Goals in the Villages of Dankassari" and the following programs in 2018-2020 and 2021-2023. This way 86 relay women and 24 relay men received initial training funded by tAESCD: 30 women in 2017, 30 women in 2018, 26 women and 24 men in 2020. Additional training was provided to 30 women in 2021, the training was delayed due to the ban on training sessions by the Nigerien government because of COVID. It finally took place from March 5 to 7, 2021, with the participation of the two family planning counselors. Moreovercomplementary training took place for 56 women and 24 men in 2022.

In Dogondouchi, 120 relay women received initial training funded by the Orsay Doutchi association: 60 in November 2019, 30 in 2020, and 30 in 2021.

In 2023, 30 relay men were trained in Dogondoutchi and the 60 relay women initially trained in 2019 underwent a refresher session.
The 2 animators from Dankassari and Dogondoutchi also participated in an update training, along with the 2 newly recruited animators in Dogonkiria and Matankari, thus creating a network.

Tools: illustrated charts and filmed sketches
At the request of those trained, an image box (a set of 27 illustrated charts) was created, along with four filmed sketches titled “Akwai Magana! Let's talk about it!”.
Due to the success of these highly educational films, four new sketches covering additional themes were produced: “Akwai Magana! Let's talk about it!” (Season 2).

First effects

Efforts to change mindsets are reflected in the statistics. According to the Niger Demographic and Health Survey 2017, the birth rate decreased from 7.6 children per woman in 2012 to 6.0 children per woman in 2017. The decline is more significant in the Dosso region, which includes Dankassari and Dogondoutchi, dropping from 7.5 to 5.7 children per woman.

From 2018 to 2020, Maimouna Kadi, counselor, met over 11000 persons, including about 35% men in the Dankassari rural community, in several dozen villages. She also took part in the registry awareness campaign early in January 2021, which led her to travel to remote areas where she can't use a motorbike, such as Karki Malam. From 2021 to 2023, she visited 5 villages three times each month, meeting an average of 300 to 400 people per month, mostly women. The focus is on visiting remote and previously unvisited villages.

Working in contact with the head nurse of the Integrated Health Center (CSI) in the commune of Dogondoutchi, facilitator Rekia Toumane visited several villages in the Dogondoutchi department in 2018, seeing over 2,300 people in 8 months, an average of nearly 300 people per month, with about 40% men. In 2019, she visited 25 further Dogondoutchi villages i.e. 5 villages per month including 3 meetings per village (15 meetings per month), dealing with one or two topics per visit.

In 2020, Rekia Toumane continued her visits and met 3540 persons overall, namely 2260 women and 1280 men.

Between 2019 and 2023, she visited a total of 47 villages in Dogondoutchi at a rate of five villages per month, conducting three sessions per village (15 sessions per month) with one or two themes each time. This represents a total of 20,500 people, including 33% men.
The visits of both facilitators will continue in 2023-2024, thanks to the continued funding from local authorities in Brittany.

Extension to the Communes of Matankari and Dogonkiria

In February 2024, the team of animators was expanded with the arrival of two new animators for the communes of Dogonkiria and Matankari, thanks to funding from the Agence Française de Développement (AFD), managed by RAIL (Réseau d'Appui aux Initiatives Locales). Regular monitoring missions for the four facilitators are conducted by health professionals and mission officers from RAEDD and RAIL. The animators report monthly, and it appears that each of them meets over three hundred people (two-thirds women and one-third men) in five different villages every month.

The activity is now managed by RAEDD and RAIL, with partners AECIN, AESCD, and the Orsay-Doutchi Exchange Association. A joint account is managed by RAIL and RAEDD, and a quarterly meetings via WhatsApp allow all partners to review progress and make decisions by mutual agreement.

In Dogondoutchi and Dankassari, animators supervise during village meetings activities carried out by relay persons (mostly women, but also men) previously identified by them and trained for this purpose. These relays use as a means of communication, tools such as “image boxes” (plasticized illustrated pagivolts) which they are permanently equipped with and continue to raise awareness at various village gatherings, including weddings, baptisms, water points, under shade trees, and women's group meetings.
In Matankari and Dogonkiria, the animators ae working to identify relay persons who will be trained at the beginning of 2025 and also equipped with pagivolts.

Since the end of 2024, the four facilitators have also been equipped with digital tablets. This new tool is proving invaluable during the facilitators' visits to the villages.

Thanks to funding from Région Bretagne in 2023-24 and Conseil Départemental Ille et Vilaine (CD 35) in 2023, AECIN and AESCD were able to pay the wages of the Dogondoutchi and Dankassari animators. Unfortunately, the hoped funding from the CD 35 was not obtained in 2024, as Niger was no longer eligible for their call for projects, and it was with lower than expected resources that the family planning action continued. It will come to an end at the end of 2024 for the monthly missions of the two animators in Dogondoutchi and Dankassari.

The mutualization of funds between AECIN, AESCD and the association d'Echanges Orsay-Doutchi, financed by AFD and the Conseil Départemental 91, has allowed us to carry out the program planned for 2024, in particularly the recycling training courses for the 60 Dogondoutchi relay persons, which took place in May 2024, and the training courses for two groups of 30 new relay persons from the villages of the Dogondoutchi and Dankassari communes, which have been carried out during the last trimester of 2024. In March 2025, two groups of 30 new relay women from villages in the communes of Dogoniria and Matankari were trained.

Project reduction due to lack of funding

At the beginning of 2025, the two facilitators of Dogondoutchi and Dankassari did not see their contracts renewed. However, support from Soroptimist will allow them to carry out some follow-up missions.

Priority is given to the two facilitators of Matankari and Dogonkiria who continue their activities who continued their work until June 2025.

The trained relay people form a network of over 320 women and me in villages in the communes of Dankassari, Dogondoutchi, Dogonkiria, and Matankari. They work continuously to raise awareness about modern contraception, the widespread availability of prenatal and postnatal care and assisted childbirth in health facilities, the fight against early marriage, and the enrollment and retention of children, especially girls, in school.

Appointments of doctors in rural areas were made in 2012 by the State of Niger.

Those doctors require basic equipment to work efficiently.
A project supported by the Brittany Region and by Tarbiyya Tatali's own resources is destined to buy basic equipment, namely: office equipment (table, chair, calculator, metal cupboard, files, paper, stapler, paper punch, clock), medical equipment (examination table, dressing kit, birth kit, diabetes meter, blood pressure meter, scales for adults, steel tray).

The cost of equipping one doctor amounts to 1,500 euros.

The first doctors have been appointed at integrated health centers in Dankassari,

Thanks to the Total Foundation, the doctors in Kieche, Guecheme and Kore Mairoua have also been equipped.

Unfortunately, only 3 doctors were still present in the Dogondoutchi Department in 2018: two in the Dogondoutchi hospital and one in Dankassari.

By the end of 1919, only one doctor remains in the department of Dogondoutchi, appointed at the Dogondoutchi hospital.

In 2020, 1500 health staff including 250 doctors have been engaged, owing to the COVID crisis, and 6 doctors have been appointed in the Dogondoutchi departement : 3 to the Dogondoutchi hospital a lady doctor at Dankassari, one at Dogonkiria and another at Soucoucoutane.

Malnutrition is a chronic problem in Niger, it is estimated that nearly 30% of Niger's children suffer from it in varying degrees.

During the food crisis in 2005

Severely undernourished children are hospitalized in a recovery center created by Helen Keller International (HKI) in 2005. 176 children were treated in this center from August 2005 to February 2006. Tarbiyya Tatali found out that the mothers, living close to their babies in the hospital were also undernourished and needed help. Tarbiyya took responsibility to feed them for the first forty five days, and then HKI took over. A bar of soap has also been supplied weekly to these mothers.

Distributing Spiruline

The alga treats child malnutrition efficiently with just two to five grams per day per child. But growing spiruline is costly (18,000 CFA, 27 euros per kilo), and Nigeriens are not accustomed to using it.

Since the spiruline farm of RAEDD started, in 2009, 20% of the production has been donated every month as humanitarian aid to the Dogondoutchi hospital

Physiotherapy is under-used in Niger. Within the country, only ten or so National and Regional hospitals are endowed with a functional rehabilitation ward. A Physiotherapy School does exist in Niamey, five students will graduate there in 2013. Our project aims at helping the most disadvantaged patients by enabling them to receive rehabilitation care.

The birth of the project.

Sabrina, Sandra and Camille volunteered from January to March 2008 at the capital's Niamey National Hospital. They worked in Traumatology common wards, treating non-paying patients. Such wards lack physiotherapy treatment, which is direly needed. Hence, the project was hatched at their return.

The goals of the project.

The main goal is to ensure treatment in traumatology common wards.

Four Nigerien physiotherapists have followed one another since December 2009: Mahamadou, Diadié, Maman Sani, and Kadidja since March 2017.

Assessment of 2009-2010

So as to maintain follow-up care, a Nigerien physiotherapist has been engaged by the association in December 2009. His contract ended and he has been replaced by a young physiotherapy graduate: Diadie.

Diadie

French physiotherapists came to second him in treating patients and they complement his theoretical and practical training. The feasability of tutoring Nigerien physiotherapy interns is under study. French physiotherapists could also assist the teaching staff of the Niamey Physiotherapy College.

From October 2010 to January 2011, French physiotherapists have been teaching on Friday afternoons at the Niamey School of Physiotherapy. A tutoring scheme for Nigerien physio interns is being studied. In the National Hospital, an in-job training mission has been launched, enabling working physios to improve their skills thanks to practical work.

Since January 2011, the association has relied on the employment of a Nigerien physio, as solidarity stays of French physios have been stopped for safety reasons.

Assessment of 2011-2012

The association has engaged a Nigerien physiotherapist and now relies entirely on him, as volunteer missions have all been cancelled since Antoine and Vincent's death, following their abduction in Niamey on January 8th, 2011. The safety of French nationals being at risk, resuming such missions is out of the question at the present.

Concerning the funding of the project, our sole expense is Diadé's wages. Three months' wages have been covered by the Breakfast action launched in September by Nicolas, a physiotherapy student in Rennes.

Diadé's monthly wages, 50,000 CFA (80 euros) are still entrusted to AECIN, but there is a hope that he could be incorporated into Niger civic service scheme, or, as a health worker, be integrated into the civil service.

Diadié's status has changed, he started a two-year civilian internship within the hospital in June 2012. He left the Traumatology – orthopedics- neurosurgery ward and joined the hospital's functional rehabilitation ward. We hope that after his civilian service he will recruited by the Niger State so as to reinforce the hospital's rehabilitation team.

Assessment of 2013-2016

On February 2013 he was replaced by Maman Sani, a Niamey Physiotherapy School graduate, who practices in the common wards, pursuing the mission in favour of the patients who can't afford the physiotherapy treatment they require.

Maman Sani's status has now changed, he began his two-year civic service within the hospital in December 2013. Unlike Diadié he remains in the neurology, trauma-orthopedics ward, which enables him to pursue his work in the common wards. We hope he will be recruited by the Niger State after his civic service, which would acknowledge our association's work to promote physiotherapy for everyone.

His salary will now be paid by the Niger State, as soon as the funds are allocated. We will top up his salary, so as to put forward his involvement in the project and in drafting monthly reports, which enable us to keep in touch with the project and to communicate with the physiotherapist.

In 2015, the purchase of technical equipment (2 walking frames, a pair of axillary crutches and a pair of forearm crutches) was added to that monthly expense.
The physiotherapy project is funded by AECIN from its own resources.

Moreover, four students of the Physiotherapy College in Rennes got in touch with AECIN during the 2016 first term in view of actions in favour of Niger.

After his civic internship Maman Sani wished to proceed professionally and did not extend his mission with us.

Early in 2017 RAEDD engaged a new physiotherapist, Kadidja, so as to maintain a rehabilitation program in the Niamey Hospital common wards.

Kadidiatoua's appointment in Niamey's Main General Hospital led her to leave the Hospital's common ward in March. She recommended another physiotherapist to replace her. He is Goumar Abdoul Aziz who started practising in the common ward on May 3rd, 2019. As was the case with Kadidiatou, his salary is financed by AECIN's own resources.

The action was stopped in March 2020, as AECIN's own ressources were needed for other actions,

The matrons are women who help with deliveries. They are trusted by the population, including women. Most of the time, they haven't been trained at all.

Training matrons is a public health priority

In Niger, the health of women and children is of great concern. Many women continue to die in childbirth.

Our survey on health cabins in the Dogon Doutchi area revealed that the training of matrons is a priority for both health.prefessionals and population.

Aims of the training

This training will help combat maternal and child mortality, so that matrons know how to detect high-risk pregnancies and deliveries going wrong.

Through training on contraception, they will be able to inform women of modern methods of controlling birth..

Organization of training

Two-week training sessions are organized in Dogondooutchi. On completing her training, each matron is given a kit including equipment (wood case, towel, storm lantern, torch, thread, kettle, mat, bucket, draw sheet) and consumables (gloves, razor blades, thread, soap, antiseptics, cotton wool, compresses, betadine (1 bottle) , argyrol.

In total, 73 matrons have been trained from 2008 to 2012.

Training from 2008 to 2012

In 2008 we have trained a group of 12 matrons of the town of Dankassari, as we have not received the funding that we had requested to train 150 matrons from the department of Dogon. Doutchi. This action is in partnership with the association "Fredie la vie au Niger".

In 2008-2010, 20 new matrons were trained. 10 in the Dankassari community thanks to the decentralized Cesson-Dankassari cooperation, 10 in the Dogon-Doutchi rural villages thanks to a new donation from the charity ‘Fredie la vie au Niger '.

Early in 2011, 16 new matrons from villages in the Soukoukoutan and Dogonkiria communities were trained thanks to funding from Rennes City Council.

Early in 2012, the Rennes City Council and the charity ‘Fredie la vie au Niger' funded the training of, another 25 matrons from villages associated to Dankassari, Dogon Doutchi Nord, Doumega Guecheme, Kiéché, Koré Mai Roua, Lido, Matankari, Tibiri.

Thanks to the Cesson-Dankassari cooperation, some of the matrons can use an ox-drawn cart in case of difficult deliveries.

An assessment of our Matron Training Scheme was published in our booklet ‘Life Giving in Niger' on May 13th 2012.

It happened that in some villages old matrons had been replaced by younger ones who had never benefited from any training.

A further two-week training session took place for ten matrons of Dankassari in december 2017 and another, also for ten matrons of Dankassari, in July 2020. The twenty matrons were also fitted out with kits

Twenty new matrons were trained in Dankassari, in October 2021 and July 2022. They asked to be equipped with a professional gown, which is yellow in color.

Providing carts and oxen.

Some of the matrons have an ox-drawn cart, allowing them to evacuate patients particularly in case of difficult deliveries; it also improves their income.

The action was initiated by the Decentralised Cesson-Dankassari Cooperation..
Thirty-nine matrons were also similarly equipped from 2011 to 2020 (mostly thanks to the matrons themselves repaying the carts.). By 2025, the number of carts allocated is forty-seven.

The destitute population, faithful to the Azna traditions is very sensitive to “lahiya”- health – the supreme asset. The cabin eventually started with a health agent paid by the State.

Follow up

Tarbiyya Tatali visits this health hut regularly and supplied medicines twice for a total amount of 100 000 CFA (150 euros).

Unfortunately it appeared en 2006 that the lack of motivation of the health agent penalizes the services of the health cabin of Lougou.

Another better motivated health agent has been identified and he receives a monthly premium from Tarbiyya Tatali to reward his efforts for the village.
